What is EFMP?

EFMP stands for Exceptional Family Member Program.  An exceptional family members is a member (child or adult) with any physical, emotional, developmental, or intellectual disability that requires special treatment, therapy, education, training, or counseling. 

How do I enroll in EFMP?

There are 2 forms required to enroll in EFMP, DD 2792 and DD 2792-1.  Form 2792 is the family member medical summary, to be completed by a qualified medical professional.  Form 2492-1 is the special education summary to be school staff.  Once the forms are completed, they are to be submitted to the EFMP office at David Grant Medical Center (Mental Health Clinic) to be reviewed for final enrollment.  Please see below for copies of the required documents.      

As a Newcomer to Travis AFB, where do I go first to get EFMP information, resources and connections?

Your first stop should be to the AFRC, where the EFMP-FS Coordinator is located.  Sue Tonarelli can be reached at 707-424-2486 or by e-mail at susan.tonarelli@us.af.mil.  The EFMP-FS Coordinator can give you resources, valuable local information, support, and a list of current activities and events.

How do I update my EFMP Paperwork?

It's suggested that Forms 2792 and 2792-1 are updated if the needs of the family member change, new forms are issued, and on average every 3 years.  Print the forms by clicking the form links below, have your doctor complete them, and submit to the EFMP office at David Grant Medical Center, located in the Mental Health Clinic on the 1st floor.  The contact there is Capt. Lay, 707-423-5157.

How do I work with Housing and Balfour Beatty?

If you are enrolled in EFMP and need specific housing or modifications on-base, please see the form below.  A TriCare medical provider will need to complete the 2nd section of the form.  This request can be used for families just arriving to Travis and needing on-base accomodations.  Please note, this doesn't guarantee preference or approval. Once the form is complete, submit to the the Balfour Beatty Communities office at the Travis AFRC.

How can Air Force Aid Society assist?

You can contact our Air Force Aid Officer at the AFRC, 707-424-2486 for information on available programs for EFMP families.

Checklist and Contacts For Travis Special Needs Families:

 

ü  Contact SNIAC (Special Needs Identification and Assignment Coordinator) to confirm enrollment in the Exceptional Family Member Program (EFMP) or become enrolled.  SNIAC enrollment is mandatory for active duty sponsors whose family members meet enrollment criteria established by DoD and U.S. Air Force policy.  The SNIAC office is located at David Grant Medical Center, 1st floor in the Mental Health Clinic.  The contact is 707-423-5168.

 ü  Call North Bay Regional Center at 707-256-1100 to set up an appointment to determine what services you can receive for your child.  There is typically at least a month wait, so it is good to call as soon as you arrive at Travis.  North Bay Regional Center is a state organization with the ability to provide services at no cost to you.  http://www.nbrc.net/about.html#services

 ü  Military One Source is a wonderful source for information.  Call them at 1-800-342-9647 anytime.  They will listen to your individual situation and direct you to local services.  They have consultants ready to assist you 24/7.

 ü  If you have school-aged children, it is recommended you contact Travis Unified School District prior to arrival at Travis AFB.  The program coordinator is Barb Fraser at 707-437-8216 or bfraser@travisusd.k12.ca.us.  Please see the Education page for additional info on schools.

ü  Tricare ECHO (Extended Care Health Option) is a supplemental program to the basic Tricare and can assist with additional services for a small copay, based on rank.  You must be enrolled in the EFMP before applying for Tricare ECHO.  There are a lot of benefits to the program based on diagnosis and need. 

For example, if your child is diagnosed with Autism, you can receive ABA (Applied Behavior Analysis) in your home through the Autism Demonstration Project http://www.tricare.mil/mybenefit/home/overview/SpecialPrograms/ECHO/AutismServicesDemonstration?
